All 1984 Contracts

On February 1, 1984, a lease agreement was approved with the Salt Lake Ensign, Emigration and Salt Lake Central Stakes of the Church of Jesus Christ of Latter-Day Saints and the Corporation of the Presiding Bishop of the Church of Jesus Christ of Latter-Day Saints for the Lindsey Gardens softball diamond near 10th Avenue and N Street. This lease gives the stakes the permission to develop the diamond into a lighted softball park in return for a guaranteed limited usage for the next 10 years.  C 84-1

 

On January 17, 1984, an interlocal agreement was approved with Salt Lake County for repair of the Big Cottonwood Creek Water Treatment Plant Bypass Channel, the Parshall Flume, which was damaged by recent flooding.  C 84-2

 

On February 15, 1984, a Cooperative Agreement was approved with the Utah Department of Transportation (UDOT) for Project M-000S(9), Signal Coordination - Various Urban Routes. This will provide for the design and construction of an urban system project to be financed in part from federal-aid highway funds. The project will provide for coordination of traffic signals on 900 West, 2100 South, 2100 East and 2300 East. The city’s matching share of the cost is $690.  C 84-3

 

On January 17, 1984, an agreement was approved with the Utah Department of Community and Economic Development in regard to the Disaster Relief Fund. This will provide grant funds to eligible applicants to assist in the repair, reconstruction, etc. of public facilities which were destroyed by flooding or mudslides. The state’s share of funds owed is $164,859.  C 84-4

 

On January 17, 1984, an agreement was approved with the Utah Department of Community and Economic Development in regard to the Disaster Relief Fund. This will provide grant funds to eligible applicants to assist in the repair, reconstruction, etc. of public facilities which were destroyed by flooding or mudslides. On January 10, 1984, an agreement was approved with Service Network, Inc. to provide inventory control consulting services for the Salt Lake International Airport.  C 84-6

 

On January 10, 1984, an agreement was approved with Joseph D. Sanders to lease Shade Hangar 2, Row 13 at the Salt Lake International Airport.  C 84-7

 

On January 10, 1984, an agreement was approved with Glen Richman to lease Shade Hangar 1, Row 12 at the Salt Lake International Airport.  C 84-8

 

On January 10, 1984, an agreement was approved with I. H. C. Hospitals, Inc., Technical Services Division, to provide maintenance-for the Salt Lake City Fire Department’s emergency medical service equipment at a cost of $3,500 per year.  C 84-9

 

On January 10, 1984, an agreement was approved .with Kenneth O. Melby for Watermain Ext. SC-GD #29 to serve property at 4840 South Highland Circle. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-10

 

On January 10, 1984, an agreement was approved with Compu-Lease for Watermain Extension RD 27-EX-2#9 to serve property at 1115-35 East South Union Avenue, Phase I. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-11

 

On January 10, 1984, an agreement was approved with the Salt Lake County Cottonwood Sanitary Sewer District to lay a sewer line under the Little Cottonwood Conduit in conjunction with the Little Willow Subdivision located at 8575 South, west of Wasatch Boulevard.  C 84-12

 

On January 10, 1984, an agreement was approved with First Security Bank of Utah for Watermain Extension 33-C-1247 to serve Cliffwood Park Subdivision, 3560 East 4790 South. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-13

 

On January 10, 1984, Special County Contract No. 835, Watermain Extension 33-C-1293 was approved with the Bishop of the Salt Lake City Diocese Catholic Church for St. Thomas More Parish, 3010 Creek Road. The watermain extension has not been completed at this time. This work will be done under the program for the extension of watermains beyond the city limits.  C 84-14

 

On January 10, 1984, an agreement was approved with the Bishop of the Salt Lake City Diocese Catholic Church for Watermain Extension 33-C-1293 to serve the St. Thomas More Parish, 3010 Creek Road. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-14

 

On January 10, 1984, a Private Street Name Agreement was approved with Michael Jaenish establishing Gambol Place as a street name for a private thoroughfare at approximately 1615 South, 1000 West to Riverside Drive. The street name is required to address individual parcels of real property which are not adjacent to a public street right-of-way.  C 84-15

 

On January 3, 1984, an agreement was approved with Louis Brown (consultant) providing for the organization of comprehensive fire fighting services at the Salt Lake International Airport. Under this agreement, the consultant will review the capabilities of Fire Station #11 at the airport and recommend actions to fully develop the station for first response structural fire fighting, recommend actions to ensure compliance with the FAA requirements for crash, fire and rescue (CFR) services at the airport, develop emergency medical services and conduct fire inspection and prevention services. The term of the agreement is for six months, expiring June 30, 1984. Cost for the services is $22.50 per hour not to exceed a total of $23,000.  C 84-16

 

On January 3, 1984, an agreement was approved with the Utah State Department of Public Safety for Lieutenant R. H. Nievaard to take a city vehicle to use while attending the F.B.I National Academy at Quantico, Virginia during the period of January 4, 1984 through March 28, 1984.  C 84-17

 

On January 3, 1984, a contract was approved with Sumner G. Margetts and Company, Inc. for the City Creek Canyon Subsurface Drainage System and Bank Stabilization, Project No. 47-Q-17. The total cost for this project is $7,680 which will be charged to flood costs. The City Engineers Office is authorized to issue a notice to proceed to the contractor.  C 84-18

 

BURROUGHS CORPORATION

Agreement addendum with Burroughs for maintenance. This establishes maintenance coverage for the 0F2000-2 OFIS file machine and terminal for a total monthly charge of $464.00.  C 84-19

 

BURROUGHS CORPORATION

Addendum to the maintenance agreement with Burroughs for the mainframe for guaranteed maintenance coverage on an “on-call” basis during holidays. The cost for this service is $250.00 per month.  C 84-19

Amendment adding new programs and deleting old programs to the existing contract.  C 84-19

 

BURROUGHS CORPORATION

Maintenance agreement addendum for coverage on the new Burroughs disk subsystem in the amount of $2,680.96, therefore, saving the City $2,300.00 per month.  C 84-19

 

On April 23, 1985, an amendment was approved with Burroughs for various changes which will mean a net decrease of approximately $579 per month.  C 84-19

 

On May 14, 1985, an amendment was approved with Burroughs Corporation to upgrade the 207 disk subsystem. The total cost for the new disk subsystem will be $9,714.88 for a total net decrease of $894.00 per month.  C 84-19

 

On March 26, 1985, an amendment was approved in the amount of $39.40 per month with Burroughs Maintenance which establishes coverage on the B22 Memory.  C 84-19

 

On February 19, 1985, an amendment was approved with Burroughs Corporation for the order of one 87369 Quad Line Adapter for Information There will be an installation charge of $80.00 and the contract will increase approximately $73.00 per month.  C 84-19

 

On January 8, 1985, an addendum was approved to the Burroughs Maintenance agreement for Information Management Services. This addendum establishes coverage on two B26 Work Stations, Processor, and Disk Drives for a monthly charge of $137.80.  C 84-19

 

On December 11, 1984, two addendums were approved for the Burroughs Maintenance Agreement which establish coverage on the B9290 Laser Printer and Memory for a total monthly charge of $1,339.71 and coverage on the B22-CP Work Stations, Disk Drive and Memory for a total monthly charge of $302.24.  C 84-19

 

On October 2, 1984, an amendment to the agreement with Burroughs Corporation was approved enabling the city to remove an old printer and replace it with a new one, order additional software and additional capacity for the Data Communications Network.  C 84-19

 

On July 23, 1984, an agreement was approved with Burroughs Corporation for maintenance coverage on all new equipment which has been installed as well as existing equipment for a monthly cost of $26,091.81.

On May 8, 1984, an amendment to the agreement with Burroughs Corporation was approved to purchase 20-ET1100 Ergonomic Workstations with keyboards for $1,895 each.  C 84-19

 

On January 6, 1984, an amended agreement was approved with Burroughs Corporation for the addition of new equipment needed and the deletion of certain equipment. The terms and conditions of the agreement are the same as per the original agreement of February 15, 1974 and amended April 5, 1979.  C 84-19

 

On January 10, 1984, the contract with Underground Construction Inc. was approved for the Tank and Pump Piping Equipment Installation and Appurtenant Work at the Salt Lake International Airport, Project 19-A-122-24. The amount of this project is $324,901. The Airport Authority is authorized to issue a notice to proceed to the contractor.

On July 12, 1984, an agreement was approved with the Salt Lake Legal Defenders Association providing legal services to indigents.  C 84-21

 

On April 17, 1984, an agreement was approved with the Salt Lake Legal Defenders Association to provide grant monies to finance legal counsel for the defense of indigent persons charged with violation of city ordinances.  C 84-21

 

On January 17, 1984, a grant agreement was approved with Salt Lake Legal Defender Association to provide grant monies to finance legal counsel for the defense of indigent persons charged with violation of city ordinances under specified procedures and financial limitations.

NEIGHBORHOOD HOUSING SERVICES, NHS OWNER-BUILT CONSTRUCTION LOAN AGREEMENT

Third amendment, extending the repayment date of $70,710.00 in Owner-Built Housing loan money to December 31, 1985. The city is retaining its lien on one home and two lots until the loan is repaid.  C 84-22

 

On May 7, 1985, an amendment was approved with Salt Lake Neighborhood Housing Services, Western Savings and Loan Company and Zions First National Bank extending the repayment date of $70,710.00 in Owner-Built Housing loan money to June 30, 1985. This is necessary because one owner-builder family was not able to qualify for financing and two youth-project constructed lots have not been sold yet. The City will retain its lien on one home and two lots until the loan is repaid.

On April 9, 1985, an amendment was approved with Neighborhood Housing Services, Inc. (NHS), Western Savings and Loan Company and Zions First National Bank for the NHS Owner-Built Contract. This amendment extends the term of the loan of $750,000.00 made to NHS from March 31, 1985 to April 30, 1985.  C 84-22

 

On January 20, 1984, a Construction Loan and Guaranty Agreement was approved between the city, Salt Lake Neighborhood Housing Services, Inc., four commercial banks, three savings and loan associations, and Western Savings and Loan Company and Zions First National Bank as agents for the NHS owner-built construction of eighteen homes near 1000 South Navajo Street. The agreement provides up to $750,000 in CDBG funding which will be returned to the city within fourteen months at 3% interest.  C 84-22

 

On January 20, 1984, an agreement was approved with the Redevelopment Agency (RDA) of Salt Lake City establishing their 1983-84 CDBG budget and defining the services to be provided for those funds.  C 84-23

 

On February 27, 1984, an amendment to the agreement with American Linen Supply Co. was approved which will correct the fee from $48.80 per month to $48.80 per week.  C 84-24

 

On January 17, 1984, an agreement was approved with American Linen Supply Co. to provide uniforms for the General Services Department. The cost is $48.80 per month effective December 15, 1983 to December 14, 1984.  C 84-24

 

On January 17, 1984, an agreement was approved with Horrocks Engineers and John Carollo Engineers to provide the city with engineering services for improvements at the Water Reclamation Plant, 1850 North Redwood Road.  C 84-25

 

On January 17, 1984, an agreement was approved with Little America Hotel Corporation for a waiver of installation of public way improvements at 400 South Main Street.  C 84-26

 

On January 17, 1984, a revocable permit was approved with Hedman Investment, Inc. for piping a section of the East Jordan Canal for the purpose of building a parking lot and providing access to Mr. Hedman’s property.  C 84-27

 

On January 24, 1984, a cost sharing agreement was approved with Salt Lake County wherein they will provide $60,000 to the city for the purpose of administering and engineering the Central City drainage master plan, Project No. 47-Q-1.  C 84-28

 

On February 27, 1984, a Contract for Professional Services Landfill Use Evaluation was approved between the city, Salt Lake County and Emcon Associates, Inc. to provide for inspection of the sanitary landfill jointly owned and operated by the city and county and to provide a technical overview of its compliance with geotechnical and engineering design constraints of plans completed by Emcon in 1981. The total amount payable under this agreement is $4,090 plus additional time and travel that may be required.  C 84-29

 

On February 27, 1984, an agreement was approved with UDOT Utah Department of Transportation for RRS-RRP-0035(21), Railroad Crossing Improvements at 900 South and Redwood Road. It is necessary to relocate fire alarm cables to a higher location due to new automatic flashing light signals with gates being installed.  C 84-30

 

On April 5, 1984, a Soils Regeneration Agreement was approved between the city, Salt Lake County and E. T. Technologies, Inc. wherein E. T. will operate a non-hazardous waste processing facility to dispose of difficult to manage wastes.  C 84-31

 

On January 24, 1984, an agreement was approved with Myron Cook and Cook Enterprises to lease Shade Hangar 18, Row 13 at the Salt Lake International Airport.  C 84-32

 

On January 24, 1984, an agreement was approved with Gaylon Pierce to lease Hangar 19, Row 1 at the Salt Lake City Airport II.  C 84-33

 

On January 24, 1984, an agreement was approved with David J. Janikowski and Buck Energy Corporation to lease Shade Hangar 20, Row 13 at the Salt Lake International Airport.  C 84-34

 

On January 24, 1984, an agreement was approved with Robert Horne to lease Hangar 3, Row 8 at the Salt Lake International Airport.  C 84-35

 

On January 23, 1984, an agreement was approved with Control Data Corporation for Login, a local government information network service. The only charges for this service will be a terminal maintenance charge of $175 and a non-refundable terminal and installation charge of $275.  C 84-36

 

On January 24, 1984, a grant agreement was approved with Neighborhood Housing Services, Inc. providing for $215,000 in CDBG funds during FY 1983-84 for their Revolving Loan Fund and administrative expenses.

On January 24, 1984, an agreement was approved with Old Dominion Resource Management Company for all work necessary to effect the recovery of overpaid FICA taxes which were paid on sick pay in accordance with the rules and regulations of the Internal Revenue Service. The city will pay Old Dominion 29.85% of credits recovered from the federal government.  C 84-38

 

On January 24, 1984, an agreement was approved with the University of North Carolina at Charlotte allowing the city to receive all broadcasts relating to “Telecon ’84-Local Governments in the 1990’s and Beyond”.  The city is obligated to pay $3,800 to UNC for the rights to receive the Teleconference but actual costs will be minimal as several local organizations are contributing money to defray the expenses.  C 84-39

 

On January 24, 1984, an agreement was approved with Triad of Utah to make an 8-inch automatic sprinkler connection for the North Plaza Office Building located at 305 West North Temple.  C 84-40

 

On January 24, 1984, an agreement was approved with Kesler Company, Inc. for Watermain Extension 33-C-1171 to serve Heughs Canyon Plat A Subdivision. This agreement covers special pumping, reservoirs and supply lines as required to furnish culinary water to this subdivision over and above the normal expenditure for the extension.  C 84-41

 

On February 14, 1984, an agreement was approved with Holiday Inn and Ronald S. Jeffrey for a 4-inch automatic sprinkler connection for the Holiday Inn, 1669 West North Temple.  C 84-42

 

On January 24, 1984, an amendment to the agreement with Snowbird, Salt Lake County Service Area No. 3 was approved amending Paragraph 4 of the 1972 agreement changing the retail price of water sold to Snowbird from one and one-half times the standard city water rate to the standard city water rate.  C 84-43

 

On January 24, 1984, an agreement was approved with Artspace Corporation for a 4-inch automatic sprinkler connection for Artspace Unit #3, 351 West Pierpont Avenue.  C 84-44

 

On January 24, 1984, an agreement was approved with the Boyer Company, H. Roger Boyer for a 6-inch Detector Check Valve connection for the Kingswood Apartments, 1300 North Redwood Road.  C 84-45

 

On January 24, 1984, an agreement was approved with the Semling-Menke Company of Utah, Inc. for a waiver of installation of public way improvements at 600 March Street.  C 84-46

 

On February 1, 1984, the contract with Associated Specialties Company was approved for Terminal Unit 1 Concourse A Wall Panel Repair at the Salt Lake International Airport, Project 19-A-86-25. The cost for this project is $10,087. The Airport Authority is authorized to issue a notice to proceed to the contractor.  C 84-47

 

On February 1, 1984, the contract with Young Electric Sign Company was approved for Miscellaneous Signs at the Salt Lake International Airport, Project 19-A-111-7.  The Base Bid and Alternate No. 1 were awarded in the amount of $41,729.75. The Airport Authority is authorized to issue a notice to proceed to the contractor.  C 84-48

 

On February 1, 1984, an agreement was approved with Stephen C. Jordan and Jordan Associates to lease Shade Hangar 8, Row 14 at the Salt Lake International Airport.  C 84-49

 

On February 1, 1984, an agreement was approved with the Salt Lake County Emergency Services Division wherein they will lease a structure once used as an ammunition bunker, for a storage facility for certain of their emergency supplies to be used in the event of an emergency in the area of the Salt Lake International Airport.  C 84-50

 

On February 1, 1984, a grant agreement was approved with Odyssey House of Utah, Inc. to provide funding for the Odyssey House Solar Demonstration project and temporary, partial funding for OH administration. The city will provide $10,000 for rehabilitation of OH’s Adolescent Program.  C 84-51

 

On February 1, 1984, a lease agreement was approved with Salt Lake County to lease a 1979 Chevrolet modular type vehicle to be used by the Salt Lake City Fire Department for the purpose of responding to hazardous materials incidents. In lieu of payment for this vehicle the city will respond to all hazardous material incidents within the unincorporated area of Salt Lake County when requested to do so.

On February 1, 1984, an agreement was approved with Victoria B. Garcia wherein she has assigned all of her rights in the Lower Millcreek Irrigation Company to the city. The city will repair the service line as consideration for the transfer.  C 84-53

 

On May 15, 1984, the contract with J and M Construction was approved in the amount of $12,567.50 for construction of Watermain Extension 35-4222, Wakara Way and Komas Drive.  C 84-54

 

On February 1, 1984, the plans were approved for the Water Reclamation Plant Retention Basin, Project 49-Q-5-34.  C 84-55

 

On February 1, 1984, an agreement was approved with Ensign Downs Incorporated providing for the installation of the water and sewer mains within Ensign Downs Plat H. EDI agrees to install the water and sewer mains as per plans approved by the Department of Public Utilities.  C 84-56

 

On February 1, 1984, an agreement was approved with Joyce P. Gill for existing Watermain Extension SC BCT 26 KR 3 to serve the property at 6100 South 1915 East. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-57

 

BEEHIVE PARKING, INC., PARKING LOT CONCESSION

Amendment No. 3 to the agreement with Beehive Parking, Inc. for the parking lot concession at the Salt Lake International Airport, decreasing the five year term of the agreement.  C 84-58

 

BEEHIVE PARKING, INC., AUTOMOBILE PARKING FACILITIES

Amendment No. 2 with Beehive Parking for management of the Airport’s public and employee automobile parking facilities. This amendment increases the fee for each employee vehicle from $6.00 per month to $8.00 per month.  C 84-58

 

BEEHIVE PARKING, INC.

First amendment to the concession agreement dated January 24, 1984 with Beehive Parking. This amendment provides for the management of two additional toll booths at the parking lot exit and alters the minimum guarantee formula so that Beehive can assume the costs associated with staffing the additional booths.  C 84-58

 

On February 1, 1984, a Bill of Sale was approved for Beehive Parking Inc. to purchase a shuttle bus to be used in conjunction with their concession at the Salt Lake International Airport.  Beehive will pay the Airport Authority’s cost of $30,015 for the 1984 El Dorado Falcon bus.  C 84-58

 

On January 24, 1984, the contract with Beehive Parking Systems, Inc. was approved for the operation of the auto parking concession at Salt Lake International Airport. The contract will be in effect for five years commencing February 1, 1984.  C 84-58

 

On February 7, 1984, an interlocal agreement was approved with Salt Lake County to provide the sum of $360,000 for the design and construction of the Parley’s Conduit, Project 47-N-270 at the following locations; 2100 South, 1000 to 1100 East; 1000 East, 2050 to 2100 South; and 2050 South, Lincoln Street to 900 East.  C 84-59

 

February 1, 1984, an agreement was approved with Salt Lake County for the survey of the Parley’s Creek Conduit, Lincoln Street and 2000 South and at the McEntyre Center, 1100 East 2100 South, Project No. 47-N-270. The study and evaluation will cost $5,400.  C 84-59

 

On February 6, 1984, a revocable permit was approved for Alma and Loa Richie allowing them to keep various fences they have erected across city property at 4th Avenue and East Capitol Boulevard.  Cross Reference P 84-13.  C 84-60

 

On February 15, 1984, Amendment No. 1 to the agreement with Barken International, Inc. was approved wherein they will lease an additional 23,123 square feet of airport property. Barken will pay 94 per square foot per year for a total of $4,009.52 per year, payable in monthly installments of $334.13. This amendment also changes the terms of the original agreement as the expiration date will be January 31, 1993. Also, in connection with this amendment a Consent of City form was approved which acknowledges the Assignment of Leasehold Estate and Transfer of Building and Personal Property that was executed between Barken and Eaton Corporation transferring the property formerly leased by Eaton to Barken.  C 84-61

 

On February 7, 1984, an agreement was approved with Barken International, Inc. to lease 25,578 feet of land at the Salt Lake International Airport for the operation of an aircraft charter and air taxi service. This lease will be in effect for eleven years at a rate of .10 per square foot per year to be increased after one year to an amount not to exceed 10% of the prior rate with another increase in five years.  C 84-61

 

On February 7, 1984, an agreement was approved with Motorola Communications and Electronics for periodic inspection and maintenance of radio communications equipment used by the Salt Lake City Airport Authority. This service will cost $1,401.20 per month and is effective from January 1, 1984 to December 31, 1985. Also, a supplemental agreement was approved to cover 17 additional hand held radios and 15 mobile radios which came off warranty on February 29, 1984. This additional coverage will cost $80.75 per month and will commence March 1, 1984 and expire December 31, 1985.  C 84-62

 

On February 14, 1984, a Professional Contractual Agreement was approved with the Utah Department of Transportation, (UDOT) Materials and Research Division, for the performance of certain construction materials tests for the Airport Authority which the city does not have the equipment to perform. These services will cost $9,000 for the 1984/85 fiscal year.  C 84-63

 

On February 14, 1984, an agreement was approved hiring Sally Anne Bishop to serve as a part-time temporary Research Assistant for the City Council. This agreement expires June 30, 1984.  C 84-64

 

On February 14, 1984, an agreement was approved with Data Destruction of Utah to provide for the hauling and destruction of discarded documents for the city with a provision that a certificate of destruction must be issued by the Police Department for each pick-up. The cost for these services will be 18ȼ per pound and the agreement will terminate January 14, 1985.  C 84-65

 

On September 18, 1984, an agreement was approved with Codex Corporation for the lease/purchase of communications modems for the Data Processing Division for a monthly cost of $1,101.  C 84-66

 

On February 17, 1984, a lease agreement was approved with Codex Corporation for the lease/purchase of eight CS48FP Modems for the Data Processing Division.  The cost will be $616 per month for 36 months.

On February 17, 1984, an agreement was approved with Triad Center for existing Watermain Extension 32K 1931 to serve the Triad Center, Phase I, Parcels 1, 6 and 7. This agreement covers special pumping, reservoirs and/or supply lines as required to furnish culinary water to this property over and above the normal expenditure for the extension.  C 84-67

 

On February 17, 1984, an agreement was approved with Kenman Corporation and Ken Holman for a 6-inch automatic sprinkler connection for the Broadway Tower Apartments, 230 East 300 South.  C 84-68

 

On February 14, 1984, an agreement was approved with Ralph S. and Jean C. Oberg wherein they have assigned all of their water rights in the Lower Millcreek Irrigation Company to the city. The Oberg’s will receive water from the city under the Special County contract and the city will repair the service line at its own cost.  C 84-69

 

On February 17, 1984, an agreement was approved with Brutger Companies, Inc. for existing Watermain Extension 35-1590 to serve a commercial motel located at 1920 West North Temple.
